What is integrated care?
As part of integrated care, the doctor/therapist and outpatient psychiatric/psychosomatic specialist services work together to provide you with support. They coordinate the treatment strategy at regular meetings and take care of your needs — even in crisis situations. You will be assigned an experienced, personal reference therapist, usually a psychiatrically/psychosomatically trained and experienced specialist, who will accompany and support you on your journey.
The specialist is your contact person and guide within the treatment team — if you need him/her, he/she is there for you. Your reference therapist is closely networked with your doctor and/or therapist and will help you with all questions relating to your treatment. Thanks to this close network, you will receive prompt help and information about necessary treatment options and ways to improve your condition and current situation. If necessary, you will receive prompt appointments from him/her. There is also a telephone service to further optimize your contact options. Your advantages: You can receive care in your familiar home environment. Even in crisis situations if required. A crisis service is available to you and your relatives at any time of day or night. Help is provided immediately, professionally and flexibly.
Participation opportunities
I as the doctor, the practice and your reference therapist work hand in hand with you and regularly discuss the treatment steps. You can take part without any complications or costs. Participation is voluntary and can be terminated at any time. Numerous health insurance companies already participate in the integrated care structure.
